Note: Registration and payment for any sessions or work with Consulting Betwixt or Dana Brinson affirms you have read and agreed to the Waiver below:

WAIVER AND RELEASE OF LIABILITY

DISCLAIMER: CONSULTING BETWIXT (THE COMPANY) AND ANY OF ITS MEMBERS, EMPLOYEES, INDEPENDENT CONTRACTORS, AGENTS, TRAINERS, COACHES, TEACHERS, INSTRUCTORS OR AIDES (THE “RELEASED PARTIES”) PROVIDING ANY SERVICES AS PART OF ANY COMPANY SESSION, TRAINING PROGRAM, ONE-ON-ONE SESSION, CIRCLE OR GATHERING, ARE NOT RESPONSIBLE FOR ANY INJURY (OR LOSS OF PROPERTY) SUFFERED BY ANY PERSON WHO IS TAKING OR HAS TAKEN A CLASS, LESSONS OR INSTRUCTION.

In consideration of participation in classes, circles, seminars, lessons, one-on-one sessions, or instruction, I hereby waive, release and covenant not-to-sue the Company and Release Parties from any and all present and future claims resulting from ordinary negligence and/or arising as a result of my engaging in or receiving instruction, guidance or training, wherever, whenever and however the same may occur. I hereby voluntarily waive any and all claims resulting from ordinary negligence, both present and future, that may be made by me, my family, estate, heirs, or assigns.

I understand that this waiver is intended to be as broad and as inclusive as permitted by law and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect.

I affirm that I am of legal age and am freely signing this agreement (or agreeing to this waiver by registering for sessions through the website). I have read and fully understand that by signing/agreeing to this form, I am giving up legal rights and remedies which may be available to me for the ordinary negligence of the Company and/or the Released Parties, or any person listed above.
